After a dental examination revealed you had periodontal (gum) disease, you began undergoing treatment. Now after several cleaning sessions, the infection has subsided and your gums have returned to a healthy shade of pink.
But your gum care isn’t over — depending on the infection’s severity you may need to visit us more often than the normal six months between regular checkups.
Gum disease arises from dental plaque, a thin film of bacteria and food remnants built up on tooth surfaces due to poor oral hygiene. The bacteria cause an infection in the gums, which initiates a response from the body’s immune system that triggers inflammation.
Without proper treatment, periodontitis can come back in which the infection spreads deeper below the gum line. Pockets of infection can reoccur as gum tissues weaken and lose their attachment to teeth. This continuing damage can ultimately lead to both tooth and bone loss.
To stop the disease it’s necessary to remove all the infection-causing plaque and calculus (hardened plaque deposits) from tooth surfaces, including around the roots. This is performed manually and could require surgery once again to access areas below the gum line.
To guard against this it’s necessary for you to undergo regular periodontal maintenance (PM). Besides cleaning, PM gives us an opportunity to check for signs of returning gum disease and, if found, plan for another round of treatment.
Although not written in stone, the interval between PM appointments that seems the most effective for preventing recurrence is every three months. In cases of advanced, aggressive gum disease, appointments may need to occur at even shorter intervals, for example every two months.
PM for susceptible patients with decreased resistance to disease require extra time and effort for the hygienist, along with a renewed daily hygiene habit of effective brushing and flossing by you to keep the disease at bay. But preventing another occurrence of gum disease and its consequences is well worth this extra attention for the health of your teeth and gums.
Advanced periodontal (gum) disease is a chronic, progressive condition characterized by bacterial infection and inflammation. Without proactive treatment, gum disease can cause extensive damage to the various tissues that hold teeth in place, and lead eventually to tooth loss.
As every war has its tactics, so the war against advanced gum disease is no different. Here’s a few of the approaches and treatments we use to stop the disease and promote healing to damaged tissues.
A Change in Behavior. Regardless of other risk factors, a film of bacterial plaque on tooth surfaces caused by a neglect of proper oral hygiene is the main culprit behind progressive gum disease. Your first step is to form new hygiene habits — brushing and flossing — that will need to be performed daily to be effective. It’s also time to end some old habits like smoking that are contributing to your gum disease.
Total Plaque Removal. Although your renewed efforts at oral hygiene are essential, it’s just as important for us to use our expertise to remove the hard deposits of plaque (known as calculus) you can’t reach with brushing and flossing. Clinging stubbornly below the gum line, these deposits will continue to be sources of infection until they’re removed. Using techniques known as scaling or root planing, we employ ultrasonic or manual instruments to access and remove as much of the offending deposits as possible. This essential step may require more than one visit to give time for inflammation to subside, and may be followed with antibiotic therapy as well.
Surgical Treatments. Although quite effective in most cases of gum disease, scaling or root planing may not be adequate in more severe cases. We still have other weapons in our arsenal, though — there are a number of surgical procedures we can use to eliminate hidden pockets of infection, or repair and regenerate damaged tissues and bone. These procedures not only help restore you to better oral health and function, but also establish a more conducive environment for maintaining future care.
Using these and other techniques, we can reduce the infection and inflammation associated with gum disease. This sets the stage for healing and renewed health, both for your mouth and your entire body.

Your body’s organ systems are interlinked — what happens in one system may affect another. An example of this is the interrelationship between periodontal (gum) disease and cardiovascular disease (CVD).
Medicine has discovered a common link between these two different conditions — inflammation. A result of the body’s defense mechanisms, chronic inflammation is damaging to both your mouth and your heart. Inflammation can destroy the gum’s soft tissue and underlying bone and lead to tooth loss. In the cardiovascular system, inflammation can begin and accelerate the buildup of plaque within arterial blood vessels (atherosclerosis). This inhibits the flow of oxygenated blood to both the heart and brain, which sets the stage for a heart attack or stroke.
Gum disease begins with poor oral hygiene. When brushing and flossing aren’t performed on a regular basis, or not performed adequately, it allows a thin layer of bacterial plaque called biofilm to build up on the teeth. The bacteria cause infection in the soft tissues of the gum that triggers the chronic inflammation. Because it’s often unaccompanied by other signs of infection like fever, a patient may not even be aware of it. There’s evidence now that inflammation caused by moderate to severe gum disease can contribute to a similar response in blood vessels.
We can treat the gum disease and reduce or eliminate the inflammation. This first requires the removal of all plaque and calculus (harder deposits) on the teeth, down to the root level. It may require surgery to access these areas and to help regenerate some of the lost tissue and bone that support the teeth. It’s also important to institute proper oral hygiene — effective daily brushing and flossing, semi-annual office cleanings and checkups.
In a similar way, you should address signs of inflammation in your cardiovascular system, including blood pressure management and the control of LDL (bad) cholesterol. Because both gum disease and CVD share many of the same risk factors, you can positively impact both your oral and general health by eating more nutritional foods, engaging in regular exercise and quitting tobacco products.
Treating any symptom of inflammation is important to improving your total health. By bringing gum disease and its accompanying inflammation under control, you may in turn help your heart and blood vessels.

TRUE. Of the hundreds of types of bacteria that occur naturally in the mouth, only a small percentage are harmful. But when oral hygiene (brushing and flossing) is lacking, these can build up in a dental plaque, or biofilm. This often causes inflammation of the gums, the first step in the progression of gum disease.
True or False: Gum disease is more prevalent among younger people
FALSE. Gum disease is most often a chronic disease, meaning that it progresses over time. Statistics show that as we age, our chances of developing gum disease increase, as does the disease's severity. In fact, according to the study mentioned above, about 70% of adults 65 and over have mild, moderate or severe periodontitis, or gum disease.
True or False: Bleeding of the gums shows that you're brushing too hard
FALSE. You might be brushing too hard — but any bleeding of the gum tissue is abnormal. Gum sensitivity, redness and bleeding are typically the early warning signs of gum disease. Another is bad breath, which may be caused by the same harmful bacteria. If you notice these symptoms, it's time for a checkup.
True or False: Smokers are more likely to develop gum disease
TRUE. Not only are smokers more likely to develop gum disease, but in its later stages they typically show more rapid bone loss. Smoking also prevents the warning signs of gum disease - bleeding and swelling of the gum tissues - from becoming apparent. Other risk factors for developing the disease include diabetes and pregnancy (due to hormonal changes). Genetics is also thought to play a role in who gets the disease — so if you have a family history of gum disease, you should be extra vigilant.
True or False: The effects of gum disease are limited to the mouth
FALSE. Numerous studies suggest that there is a relationship between periodontal health and overall health. Severe gum disease, a chronic inflammatory disease, is thought to increase the risk of cardiovascular diseases like heart attack and stroke. It may also lead to complications in pregnancy, and problems of blood-sugar control in diabetics.

Periodontal (gum) disease, though it may be invisible to everyone but your dentist, can have a powerful effect on your entire body. Not only is it dangerous to your teeth and jaws, but it can increase your risk of heart attack and stroke, cause preterm births in pregnant women, and affect blood sugar control in diabetics.
Diabetics are our subject for today. Symptoms of diabetes include abnormally high levels of glucose (a form of sugar) in the blood, leading to frequent urination, excessive thirst, blurred vision, unexplained weight loss, and loss of energy. The disease can also cause severe complications in various parts of the body.
Normally, glucose, your body's main energy source, is kept under control by a hormone called insulin, which is made by an organ called the pancreas. In type 1 diabetes, a person's pancreas does not produce enough insulin to deal with all the glucose in his or her blood. In type 2 diabetes — a condition related to increased age, physical inactivity, overweight, and heredity — the pancreas may produce enough insulin, but the body is not able to use it effectively. This condition is called insulin resistance.
People with type 1 diabetes need insulin to survive. Type 2 may be treated with exercise, diet, medications, and insulin supplements.
Serious complications of diabetes range from kidney failure, blindness, and nerve damage to infections that do not heal, gangrene and amputation of limbs.
Diabetes and periodontal disease seem to have reciprocal effects on each other. Diabetics are more likely to have periodontal disease than non-diabetics; and those with periodontal disease are likely to face worsening blood sugar control over time.
Periodontal disease (from “peri”, meaning around and “odont”, meaning tooth), is caused by dental plaque — a film of bacteria that settles on your teeth and gums every day. It's what you remove with daily brushing and flossing. Any bacteria that remain cause inflammation, which can lead in the worst cases to loss of bone and eventual loss of teeth.
The close relationship of diabetes and periodontal disease probably results from changes in the function of immune cells responsible for healing.
